Hello doctor,

I am a 46-year-old male and I wanted an opinion about my medication levels which I am currently taking. I have a prolactinoma. I am currently taking Cabergoline 5 mg twice a week. My prolactin levels have come down from 3000 ng/mL to about 500 ng/mL. I was told this is a normal level now. I am also being treated for fertility issues as I have a zero sperm count. I had a very low testosterone level but have been on Hcg 5000IU injections for the last year and Menotropins injections for the last six months. For the last three months, my testosterone and FSH (follicle-stimulating hormone) levels have been in the normal range but LH (luteinizing hormone) has been very low. I believe from what I read that HCG (human chorionic gonadotrophin) switches off LH. My sperm tests during this time have come back to zero count. I also have deep depression, which I do not know if it is a side effect of drugs or situational. What would your advice be?

I can understand your concern. From what you have mentioned it appears that you have hypogonadism and microprolactinoma. Give yourself some time, your sperm count will take some time to normalize as serum prolactin is still a bit high, and when that drops further your sperm count with be increased hopefully. Yes, these hormonal treatments can be a cause of your mood change. Try doing yoga or meditation. Exercise by way of walking for 30 minutes as it helps with symptoms of depression.
