Hello doctor,
I am a 26-year-old, and I have been masturbating on the bed for more than fifteen years.
Now I realized this does damage. In the early days of last year, when I got new bedsheets, I started losing sensitivity. The new bedsheets I have are rougher than the old ones. Now almost all my nerves in the front of the penis look scarred. There are two red dots in my pleasure nerves.
Due to the constant rubbing, I have scar tissue that looks like a burn. It has been like this for several months now. My penis has almost lost all its sensitivity. When I orgasm I do not feel it in my penis anymore (earlier I used to feel the orgasm in my penis.)
I have attached a picture. You can see the scar tissue from prone masturbation. The thing that terrifies me is whether the nerve damage can be permanent. I do not want that to happen.
I started taking magnesium supplements when I read that magnesium is perfect for nerve damage.
I hope that the magnesium supplements I take will also help heal this. I am still a virgin and I do not want my first time to be numb.
This has been bothering me for the past few months and it is only getting worse.

I am sorry to hear about your troubles. Whilst prone masturbation may give you immense pleasure, it is harmful to you in the long run.
Prone masturbation does not cause nerve damage per se, but can occasionally lead to a fracture of the penis if you are too hard on yourself.
The reason you are having these ejaculatory problems is that you are now subconsciously acclimatized to prone masturbation, which puts more pressure and excites your nerves more (giving more pleasure) than regular masturbation, oral sex, or intercourse. It is due to this feeling that most men continue to resort to prone masturbation as other forms of sexual activity fail to arouse them.
Unfortunately, there is no scientific evidence that any medication will help you reverse the sensation.
I suggest a few things to you which might help:
Stop prone masturbation and abstain from masturbation completely for a month or so. When you do restart, limit yourself to only once or twice a week, and each week use a different technique. This will help your mind readjust to the sensations and may improve your perception of orgasm.
With time, things will get better. You can continue to take any supplements, such as magnesium and vitamin E as they are all good for health even if they may not help with this issue particularly.
Once again, please do not worry. Things will improve once you stop masturbating. Give it time and follow the instructions given.

I also wanted to add a little update.
Sometime last week, while I was using my masturbator, I might have slipped or done a weird position. I had minor pain on the right side of my penis.
When I was done, I noticed the pain remained for a few hours. Later, it started coming and going. The pain was very mild. However, one day when I woke up, my penis was almost numb. I had no sexual sensitivity whatsoever, and I also noticed that my libido and sex drive were nonexistent.
I also have a hard time getting an erection now. Everything like this was a sudden onset. This has been stressing me out a lot, and I have done some research on the internet that said psychological stress can cause non-existent sensitivity. The nerves that I feel are mostly located on the front of my penis. Every time I touch it, I feel a bit of pain but no pleasure. It is like something is "blocking" my pleasure.
I had stopped performing masturbation two months ago, but I masturbated hard with my hand. I rubbed my penis against my underwear, and while doing so, I am not sure if I caused any damage. I also notice that the area to the right of my perineum, where the leg connects, has a very minor dull ache.
I also take Acetyl-L-Carnitine (2,500 mg) supplements every day and magnesium oxide supplements. I have stopped masturbation completely, and I am four days into abstinence currently. The lack of sensitivity is causing me concern because I do not want it to be permanent. I can send you some pictures of my penis with the scar if that would help.

Sure, please go ahead. But my opinion will remain the same: You have to completely stop masturbation for at least a month or so. And when you do restart, it has to be gentle and not in a prone position. I doubt if I will be able to pick something up just by looking at the photos (attachments removed to protect the patient's identity).
The fact remains that you have to give your penis a period of extended rest. As far as the pain goes, it will most likely settle down with rest. However, if you are constantly being bothered by it, you could try over-the-counter anti-inflammatory tablets for a period of three days.
Also, having a warm bath might help settle some of the pain. I would advise you strictly to give it a rest for at least a month or so, and see whether the erection and sensitivity come back before we decide to prescribe you some medications.
