Are urinary burning and hematuria prostate cancer signs?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 45-year-old male who has been experiencing severe urinary symptoms for the past three months. Having intense burning sensation during urination, difficulty starting flow, and noticed blood in urine twice this week. A digital rectal examination showed an enlarged prostate, and PSA is 7.2ng/mL (up from 2.1ng/mL last year). My family history is concerning, as my grandfather died of prostate cancer at 55, and my uncle was diagnosed at 50. Standing long hours in the kitchen becomes difficult due to frequent urination needs. MRI is scheduled for next week, but anxiety is overwhelming. Recently diagnosed with type 2 diabetes. Need guidance on advanced screening procedures and what these symptoms might indicate. Working 14-hour shifts is becoming impossible.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I can understand your concern.

Your symptoms (burning sensation, difficulty urinating, blood in urine, and rising PSA- prostate-specific antigen) are concerning, especially with your family history of prostate cancer. The enlarged prostate and PSA of 7.2ng/mL suggest a need for further investigation, though it could indicate benign prostatic hyperplasia (BPH) or possibly prostate cancer.

The MRI (magnetic resonance imaging) will help assess the prostate more clearly. Considering your recent diabetes diagnosis, it is important to manage both conditions together. While waiting for results, consider talking to your doctor about managing symptoms to make work more manageable and reduce anxiety. Based on the MRI results, advanced screenings like an MRI (magnetic resonance imaging) guided biopsy may be needed.

I hope this helps.

Kindly follow up if you have more concerns.

Thank you.
