Can prostate cancer treatments impact erectile function?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I understand that my prostate cancer requires aggressive treatment, but I am particularly concerned about preserving my sexual function. I know that treatments such as prostatectomy and radiation can significantly impact sexual health. Are there surgical techniques or strategies that could help preserve sexual function, even with the aggressive treatment my condition demands?

I have a BRCA2 mutation, a PSA level of 3.7 ng/mL, and a Gleason score of 4+4=8. Any guidance on balancing effective treatment with maintaining my quality of life would be greatly appreciated. Any advice would be greatly appreciated.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and understand your concern.

Given your BRCA2 (breast cancer gene 2) mutation and a Gleason score (a grading system used to evaluate the aggressiveness of prostate cancer) of 4+4=8, aggressive treatment is essential to address your prostate cancer (a common cancer in males that develops in the prostate gland, a small walnut-shaped gland responsible for producing seminal fluid) effectively.

However, there are strategies to help preserve sexual function while pursuing necessary treatment:

  1. Nerve-sparing surgery: A nerve-sparing radical prostatectomy may help preserve erectile function by avoiding damage to the neurovascular bundles responsible for erections. This approach depends on the tumor's size, location, and whether the cancer has spread beyond the prostate.
  2. Radiation therapy: Advanced radiation techniques, such as intensity-modulated radiation therapy (IMRT) or brachytherapy, can effectively target cancer while minimizing damage to surrounding tissues. Combining these therapies with short-term androgen deprivation therapy (ADT) may balance cancer control and the preservation of sexual function.
  3. Rehabilitation post-treatment: Engaging in early penile rehabilitation can improve sexual outcomes after treatment. Options include the use of medications like phosphodiesterase type 5 (PDE5) inhibitors, vacuum erection devices, or other therapies to promote blood flow and nerve recovery.

I hope this helps.

Please revert so I can assist you further.

Thank you.
