Hello doctor,
My 72-year-old father had his prostate removed due to cancer two months ago. Since the surgery, he has been experiencing constant pain in his pelvis and lower back. Is this typical during recovery, or could it suggest that the cancer has spread? Should we request imaging, or are there other ways to manage his pain? Any advice would be greatly appreciated.

Based on your father's symptoms and complaints, it is likely that he is experiencing post-operative hypocalcemia (low calcium levels in the blood) and neuropathic pain (nerve pain). This is a common occurrence after surgery and may subside within three to six months. Below are my recommendations, and it would be beneficial for you to follow them and review them accordingly. The possible cause can be post-operative neuropathic pain, which occurs when nerve damage or irritation after surgery causes burning or tingling pain. Hypocalcemia can cause muscle cramps and numbness.
Before deciding on the next steps, the following investigations are advisable:
Kidney function test (KFT) with serum electrolytes: To assess kidney health and electrolyte imbalances.
Serum vitamin B12 and folate levels: To check for deficiencies affecting nerve function.
NCCT (non-contrast computed tomography) of the whole abdomen: To rule out structural issues like tumors.
The following conditions need to be ruled out:
Possible metastasis: Considering the possibility of cancer spreading to the abdomen.
I suggest the following measures:
Take the tablet of Neurokind LC (Mecobalamin, L-methylfolate, Alpha-lipoic acid) two times a day for 10 days, then once a day for one month.
Take the tablet Ultracet (Tramadol and Acetaminophen) as needed for pain.
Take capsule Pan D (Pantoprazole) once a day for 10 days.
Take tablet Shelcal D3 (Calcium and Vitamin D) once a day for one month.
Apply ointment Voveran LA (Diclofenac sodium) twice daily for 10 days.
Kindly consult your doctor and take medicines accordingly.
Preventive measures include:
Follow a high-fiber diet.
Drink plenty of fluids.
Perform passive exercises.
Review physiotherapy as necessary.
