Is a prostate biopsy to detect cancer safe?

Hello doctor,

During my routine checkup, my PSA was 2.8 ng/mL, and the digital rectal exam revealed an asymmetric prostate with a firm nodule on the right side. Although I am relatively young, my urologist is recommending a biopsy. Two of my uncles had prostate cancer. I stay active, maintain a healthy weight, and have no urinary symptoms. What are the risks and benefits of having a biopsy at this stage? Should I consider getting a second opinion? Please help.

The normal range for PSA (prostate-specific antigen) levels is generally considered to be between 2.5 and 4 ng/mL (nanograms per milliliter).

Although your PSA level falls within the normal range, I agree with your doctor's recommendation for further investigation due to two key factors:

  1. The presence of a prostatic nodule (lump or abnormal growth), even if it is painless.

  2. A positive family history of prostate cancer.

In 35 percent of cases, a prostatic nodule is benign; however, in 65 percent of cases, it may be cancerous. Given this, additional tests are warranted to ensure accurate diagnosis and management.

Magnetic resonance imaging (MRI) is generally considered more accurate than a biopsy (a medical procedure where a small sample of tissue is taken from the body to be examined under a microscope) in detecting prostate cancer, as a biopsy can miss cancer in approximately 20 percent of cases.
