Hello doctor,

My arms and legs are covered with thick, hard bumps that cause severe itching, making it difficult for me to sleep. These symptoms have persisted for several months, and I find myself scratching until I bleed. My general practitioner diagnosed me with Prurigo nodularis, but the ointment prescribed has proven ineffective. I have attempted various remedies, including aloe vera, calamine lotion, and steroid cream, yet none have provided relief.

  1. Is there a more potent treatment available?
  2. Are there oral medications that might be more effective than topical creams?
  3. Also, I would like to know the cause of this condition. Could it be related to a food allergy?

The onset of Prurigo nodularis is typically not related to food allergies but is often linked to chronic skin inflammation, nerve dysfunction, or various underlying health conditions, including:

  1. Atopic dermatitis or eczema.

  2. Autoimmune disorders, such as thyroid disease.

  3. Diabetes or kidney issues.

  4. Neurological disorders may involve nerve damage or heightened itch signals.

Engaging in scratching exacerbates the condition; each scratch can thicken the skin, intensifying the itch and perpetuating the itch-scratch cycle.

In cases where topical treatments like steroids and calamine are ineffective, alternative oral medications or advanced therapies may be required, including:

  1. Antihistamines (Oral): Tablet Hydroxyzine (anti-histaminic drug) which is more potent than over-the-counter allergy medications. Antihistamines can provide relief at night. Tablet Gabapentin or Pregabalin (anticonvulsant) can also be used particularly if this is associated with nerve overactivity.
  2. Immunosuppressants (For Severe Cases): Tablet of Cyclosporine, or Mycophenolate, (immune suppressant) which are utilized in more severe instances.

  4. Additional Therapies: Narrowband ultraviolet B phototherapy, a light therapy that may assist in reducing nodules. Cryotherapy, which involves freezing techniques to diminish persistent nodules can also be used.
