Patient's Query
Hello doctor,
I am a 38-year-old woman experiencing symmetrical joint pain and stiffness in my hands, wrists, and feet that is worst in the morning and can last up to two hours before improving. My knuckles and wrists are visibly swollen, warm to the touch, and so painful that I am having difficulty with simple tasks like opening jars, typing at work, or even buttoning my clothes.
I am also feeling extremely fatigued despite getting adequate sleep, and I have noticed small, firm lumps under the skin on my forearms. My grandmother had rheumatoid arthritis, and I am concerned that I am developing the same autoimmune condition since these symptoms have been progressively worsening over the past three months. Please tell me, how to handle this?
Kindly help.
Hello,
Welcome to icliniq.com.
I read your query and can understand your concern.
Your symptoms are very similar to those of inflammatory arthritis. It is most likely rheumatoid arthritis (RA). The key symptoms are
Symmetrical joint pain.
Swelling of knuckles and wrists.
Morning stiffness lasting more than one hour.
Gradual worsening of symptoms over months.
This pattern is different from wear-and-tear arthritis. The fatigue you feel is common in autoimmune diseases. In RA, inflammation affects the whole body, not just joints, so people often feel exhausted even after good sleep. This is a disease-related symptom, not weakness.
The small firm lumps under your skin, especially on the forearms, sound like rheumatoid nodules. These are seen in RA and usually indicate active immune inflammation.
At this stage, early diagnosis is very important. I am advising you to do the following tests to confirm the diagnosis,
Rheumatoid Factor (RF).
Erythrocite sedimentation rate (ESR).
C-reactive protein (CRP).
X-rays of hands and feet.
If RA is confirmed, early treatment can prevent permanent joint damage and disability. Modern medications such as Disease-modifying antirheumatic drugs (DMARDs) can control inflammation, reduce pain and swelling, and allow people to live normal, active lives if started early.
I strongly advise you to see a rheumatologist as soon as possible. Do not delay evaluation. RA is treatable, especially when caught early, and timely treatment can protect your joints, work ability, and long-term quality of life.
I hope this information helps you.
