Should I get rabies PEP after a suspected cat scratch?

Hi doctor,

I have observed a small superficial scratch (about 1 cm) on my forehead. I sleep in a closed room, and I think it could be a scratch from a rabid cat. Can you suggest whether I need post-exposure prophylaxis (PEP)?

As the scratch was made in your forehead, a more risky area since it is near your brain, it is crucial to get medical advice as quickly as possible. As you suspected, the scratch might have been made by a rabid cat, and according to the photo, the scratch broke the skin. It is advised to visit the nearest hospital or rabies vaccination center to undergo post-exposure prophylaxis (PEP) treatment.

Even if the incident took place three days ago and you do not feel any symptoms at the moment, PEP can still be started right away. The doctor may also advise you to have rabies immunoglobulin administered, depending on the exposure and wound.

It is also recommended to consult a physician on having a tetanus vaccination after telling him about your medical background.

World Health Organization (WHO) recommended schedule for rabies vaccine:

  • First shot - Day 0 (the day when the treatment begins).

  • Second shot - Day 3.

  • Third shot - Day 7.

  • Fourth shot - Day 14.
