There are many causes of right ventricular hypertrophy and right axis deviation in the ECG (electrocardiogram). When it is combined with shortness of breath, it needs clinical investigation and diagnosis for proper treatment. I would recommend an echocardiogram and basic laboratory tests to begin with. The ECG (attachments attached to protect the patient's identity) that you have shared is not complete, though you have mentioned the findings.

  1. These findings may be seen in patients having raised pressure in their lungs due to either left heart disease or pulmonary vascular disease, or due to some blood clot within lung arteries or lung parenchymal disease.

  2. Some kind of pressure overload on the right side of the heart, such as in a condition where one of your heart valves, called a pulmonic valve, is of a narrow orifice, and in a condition with volume overload, such as intracardiac shunts.

  3. These conditions need proper investigation and treatment, as I have mentioned above. An echocardiogram and basic laboratory test are the best strategies to begin with.
