Hello doctor, I just returned from a three-week hiking trip in a rural area and have been feeling very unwell for the past five days. It started with a persistent fever (101–103°F), severe headache, and intense muscle pain. I noticed an unusual scab-like sore on my ankle that I do not recall getting, and now a rash is spreading across my chest. What is most concerning is the extreme fatigue I can barely walk to the bathroom. My travel partner mentioned that scrub typhus is common in that region. Could this be what I have? I read that it can be serious if not treated quickly. Should I go to the emergency room (ER), or can you prescribe something? I am supposed to start a new job next week and can’t afford to stay this sick.

Symptoms of scrub typhus usually begin within 10 days after infection. Signs and symptoms include:
Fever and chills.
Headache.
Body aches and muscle pain.
A dark, scab-like region at the site of the chigger bite (also known as an eschar).
Mental changes, ranging from confusion to coma.
Enlarged lymph nodes.
Rash.
If not treated promptly, scrub typhus can be fatal and may lead to organ damage and severe bleeding.
Treatment:
Capsule Doxycycline 100 mg twice daily for seven days.
Tablet Paracetamol 500 mg three times daily for five days.
Tablet Pantoprazole 40 mg once daily on an empty stomach for seven days.
Doxycycline is the drug of choice and has the best results when started early. Therefore, you must visit the emergency room (ER) immediately and begin treatment with Doxycycline.
