What causes scrub typhus despite taking Doxycycline?

Hello doctor,

My husband contracted scrub typhus after our camping trip in an Asian country last week. He developed a strange black mark on his neck, which the local doctor identified as an eschar. Now, he has a high fever (104°F), a severe headache, and is unable to get out of bed.

He began Doxycycline yesterday, but there has been no improvement. How long should it take for the medicine to start working? His blood test shows very low platelet counts. Should we be concerned about this?

Doxycycline is the first-line treatment for scrub typhus, and it typically takes 24 to 48 hours to see noticeable improvement in symptoms.

If his fever and weakness persist beyond this period, or if his condition worsens, you should take him to the hospital immediately, as he may require IV Doxycycline.

Low platelet counts are common in scrub typhus and can be concerning, as they may lead to bleeding or multi-organ failure.

  1. Watch for these symptoms, and take him to the hospital if you notice any of them:

  2. Persistent high fever or worsening symptoms despite Doxycycline.

  3. Confusion, disorientation, or drowsiness (may indicate encephalitis).

  4. Shortness of breath or difficulty breathing (may indicate lung involvement).

  5. Severe abdominal pain, jaundice (yellowing of the skin, eyes, and mucous membranes due to an excess of bilirubin in the blood), or reduced urine output (may indicate organ failure).

Please monitor his condition by doing the following tests every other day:

  1. CBC or complete count (to track platelet count).

  2. Liver function tests (LFT).

  3. Kidney function tests (KFT).

  4. C-reactive protein (CRP).

  5. D-dimer.

In the meantime, provide him with iron, Vitamin B, C, K, and zinc-rich foods. Consider giving him papaya and boiling papaya leaves to make a drink, as it may help increase platelet counts.
