Can I have epilepsy with a normal MRI at 24?
Patient's Query
Hello doctor,
I am 24 and have had two seizures within the last four months, both completely unexpected, with no prior warning or family history.
My neurologist ordered an MRI, which came back entirely normal, and now there seems to be confusion about whether to proceed with any treatment at all.
Are normal MRI results still possible with true epilepsy at 24? Because my doctor seemed almost doubtful after seeing those results, and I left more confused than before.
Should an EEG have been done immediately alongside the MRI?
What happens if another seizure occurs before my next appointment, which is still six weeks away?
Kindly help.
Thank you.
Hello,
Welcome to icliniq.com.
I read your query and can understand your concern.
Yes, it is very common to have a normal MRI (magnetic resonance imaging) in epilepsy. MRI mainly looks for structural problems in the brain. Many patients with epilepsy have completely normal scans.
Your history of two unprovoked seizures in four months is clinically significant. Even with a normal MRI, this can still be diagnosed as epilepsy depending on the evaluation.
An EEG (electroencephalogram) should definitely be done. It helps detect abnormal electrical activity in the brain. Ideally, it is done early because it increases the chance of confirming the diagnosis.
A normal EEG also does not completely rule out epilepsy, but it gives important supportive information. Sometimes, a repeat EEG or a sleep-deprived EEG is needed.
Regarding treatment, many neurologists consider starting anti-seizure medication after two unprovoked seizures, even if the MRI is normal. This is to prevent further episodes.
Until your next appointment, you should take the following precautions.
-
Avoid sleep deprivation, alcohol, and stress.
-
Do not drive, swim alone, or be at heights.
If another seizure occurs, seek medical attention immediately. If the seizure lasts more than five minutes or repeats, it is an emergency.
Please understand, your doctor is not dismissing your symptoms. An MRI being normal only means there is no visible structural lesion. It does not rule out epilepsy.
You should follow up sooner if possible and request an EEG. Early clarity will help reduce anxiety and guide treatment properly.
I hope this helps.
Thank you and take care.
Regards.
