What is the treatment for nasal septum perforation?

Hello doctor,

I have a question regarding a large septum hole and nasal crusting. I have been diagnosed with sarcoidosis before 13 years and not sure if this is causing the perforation. The perforation is nearly three centimeters in length and would like to know if using saline spray and nasal oil cease the crusting. Will this help me avoid surgery? Kindly help.

I can understand your concern.

If you do not want to get the septal perforation corrected via surgery, the only way to prevent crusting is by using nasal saline sprays intermittently during the day. I would suggest you use this 10 to 15 times a day. I would prefer nasal saline sprays rather than drops as they give more surface area contact compared to drops. If you have been diagnosed with any other co-morbid conditions like diabetes or vitamin D deficiency, I would suggest you get that part corrected for the best and long-lasting effects.

Please remember that these nasal saline sprays are used only to prevent crusting and there by any mucosa infection. a large septal perforation will never close on its own and will always require surgical intervention in order to close it.

Thank you for the reply.

Can I try removing the crust with tweezers or spray onto it? If I use the spray as you suggested throughout the day, will my nose heal, and can I stop using the saline spray? Kindly help.

I can understand your concern.

Never attempt to remove any crusts from the nose as it can lead to infection and bleeding. The sprays suggested are to keep the nasal mucosa moist so that crusts do not develop and hence will be used for a lifetime to reduce crusting. When the nasal mucosa is kept moist, most of the crusts get dislodged on their own and usually come out of the nose. As long as you breathe, there will be air currents that will get distorted in the area of perforation causing crusting again and again. It is very rare to see no crusts in anyone having perforation and not using nasal sprays or douching.
