Hi doctor,
I am 20 years old. I had just gotten my total testosterone checked, and it came back at 412 ng/dL. My total free testosterone was 7.9 ng/dL. My SHBG was also 31.7, which was the only other thing I could get looked at by my doctor. But I feel terrible; my libido is very low. I do not have that much energy throughout my day, either. I am in a poor mood a lot of the time, never happy or sad. I like to train for hypertrophy. I make sure to do 30 minutes of moderate-intensity cardio, which is walking on a 12 incline. I make sure to get 10,000 steps a day as well. My diet is good. I make sure to track what I eat. I sleep for at least nine hours every night. I am honestly just looking for some advice. I have tried my hardest to optimize my levels as best as I can, but I just feel off. Kindly guide.

Total testosterone of 412 ng/dL and free testosterone of 7.9 ng/dL are technically in range, but they are on the low end for a healthy 20-year-old, especially with symptoms like low libido, poor mood, and low energy. SHBG (sex hormone binding globulin) of 31.7 is normal, so it is likely not binding up too much free testosterone. It might be worth checking other hormones like LH (luteinizing hormone), FSH (follicle-stimulating hormone), prolactin, estradiol, and thyroid panel to rule out secondary causes. Also, consider mental health support, as mood can heavily affect libido and energy, too.
