What are the treatment options for uneven smile line?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have an uneven smile, with one side higher than the other. My left and right sides look very different. I do not know if this has to do with different-sized front teeth.

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

Review of a smile line, lips, lip competency, overbite, overjet, facial profile, and form or shape of the head -all these count towards aesthetics. These proportions need to be reviewed for proper dental classification, malocclusion type, and skeletal malocclusion if any.

In your case, the occlusal cant (occlusal plane) and lip competency have to be examined.

The probable causes include:

  1. Lip incompetence.

  2. Increased overbite and overjet.

  3. Facial contour and profile.

I suggest you to do the following:

  1. Intraoral and extraoral photographs.

  2. Orthopantomogram (OPG).

  3. Cephalometric analysis (CEPH).

  4. Pretreatment casts after clinical review.

The differential diagnosis includes:

  1. Molar relation.

The probable diagnosis includes:

  1. Angle’s classification of malocclusion.

  2. Incompetent lips.

  3. Increased overbite and overjet.

  4. Convex facial profile.

I advise you to do the following:

  1. Space evaluation.

  2. Retroclination of anterior teeth with the evaluation of lip competency.

After treatment, a follow-up has to be done.

The preventive measures include:

  1. Retention.

  2. Regular follow-up treatment visits.

Kindly consult a specialist doctor, talk with them, and take medications with their consent.

I hope this helps.

Thank you.
