What causes morning throat pain and a clogged ear sensation?
Patient's Query
Hello doctor,
I have been experiencing a sore throat with pain while swallowing, a clogged or muffled sensation in one ear, and thick phlegm in my throat, especially in the morning.
I also notice mild hoarseness and a feeling of pressure in the ear that sometimes pops when I swallow.
These symptoms are worse in the morning and gradually improve as the day goes on.
I do not have severe ear pain, fever, dizziness, or breathing difficulty. I am seeking a medical evaluation to determine the underlying cause and appropriate treatment.
Kindly suggest.
Hello,
Welcome to icliniq.com
I understand your concern.
Based on your symptoms, a sore throat with pain on swallowing, a clogged or muffled ear sensation (likely due to Eustachian tube dysfunction), thick phlegm in the morning, mild hoarseness, and ear pressure that sometimes pops, allergic rhinitis (hay fever) is the most likely cause.
Exposure to allergens such as dust mites, pollen, or mold can trigger nasal inflammation. This inflammation often leads to post-nasal drip, which causes throat irritation and thick phlegm, especially after waking.
Swelling around the Eustachian tube can result in ear fullness or pressure, and irritation of the vocal cords may cause mild voice changes.
The fact that your symptoms are worse in the morning and improve during the day supports this diagnosis, as overnight allergen exposure and mucus pooling can worsen symptoms, while daytime movement and activity help clear secretions.
The absence of fever, severe pain, dizziness, or breathing difficulty makes acute infection, sinusitis, or middle ear infection less likely at this stage.
Initial management may include an intranasal corticosteroid spray such as Budesonide or Fluticasone (Flonase), one to two sprays in each nostril once daily in the morning for one to two weeks, to reduce nasal inflammation. This treatment is highly effective for allergic rhinitis when used regularly.
In addition, use a saline nasal rinse twice daily, stay well hydrated, and reduce allergen exposure by washing bedding regularly, using air filters, and avoiding known triggers.
If there is no improvement within seven to 10 days, or if symptoms worsen, an evaluation by an ENT( ear, nose, throat) specialist is advised for further assessment and possible allergy testing.
Monitor for minor side effects such as nasal dryness or occasional nosebleeds, which are uncommon but possible.
I hope this information is helpful. Please feel free to follow up if you have any further questions.
Thank you.
