Features suggest micronutrient deficiency. You need to get some laboratory investigation done. Get CBC (complete blood count), LDH (Lactate dehydrogenase ), ferritin, ALT (alanine transaminase), and creatinine levels done.
Build up your vitamin B12, folic acid, pyridoxine, vitamin D, and iron levels. Get a course to deworm yourself.

I reviewed your reports (attachments removed to protect the patient's identity). You are severely anemic. Build up your vitamin B12, folic acid, pyridoxine, vitamin D, and iron levels. Get a course to deworm yourself.
Some things still need to be done. Get ferritin, ALT (alanine transaminase), and creatinine levels done.
