Hello doctor,

My mother got a brain stroke last month. Since then, she has been hospitalized. She got another stroke on the first week this month. We immediately did an magnetic resonance imaging (MRI) and saw a bigger clot. The earlier clot had also increased in size. So within the window period, we gave her the Boehringer Knoll injection of 60 mL which is the strongest thinner. As a side effect of this, there was brain hemorrhage and blood vomits. Blood oozed out of the stomach too. She is able to speak much better and all her limbs are working well. Comprehension is weak though. I have attached the latest MRI reports and current medicines. She is on a liquids-only diet. Please advise the next steps. Should we continue hospitalization or take her home? She is desperate to go home.

Welcome to icliniq.com. I have seen the files provided by you (attachment removed to protect patient identity). It is unfortunate that your mother developedbleeding after Boehringer Knoll injection. It is documented to happen in 5 to 7 % of the patients to whom this injection is given for stroke. In such a situation, measures to stop bleeding are done and a close monitoring of neurological status is required. Some patients may require brain surgery to relieve increased pressure inside the brain. Along with that, it is required to try to find out the risk factors of stroke, so that further long term treatment can be decided. In the attached prescription, there is noblood thinner such as Ecosprin or Clopidogrel mentioned. Possibly, your doctor will start it when she is better and the bleeding risk is reduced. I suggest you talk to the treating doctor about the decision to keep her in the hospital or take her home as that decision cannot be given on the basis of a small description alone. If you take her home, you will need to arrange a few things for her nursing care and physiotherapy. You can send all her reports, so that a long-term treatment optimization can be done.

Thank you for the prompt reply. She is being given Enoxatroy (Enoxaparin Sodium) injection daily at 3.00 PM for thinning. So, in your analysis, should we continue hospitalization?

Welcome back to icliniq.com. Injection Enoxaparin is most likely being given as DVT (deep vein thrombosis) prophylaxis. I feel in the future, she will require antiplatelet tablets (Ecosprin or Clopidogrel). Regarding continuation of hospitalization, I cannot decide that from the given description. You should talk to your treating doctor about it.
