Hello doctor,
I have had some sickness and fatigue recently after a simple workout, lasting all day, and episodes of spikes in tachycardia while at rest, and most recently, my heart rate has been going up and down at rest. I have shortness of breath when walking and doing household activities. I feel very lightheaded, dizzy, and nauseated daily, off and on. Four ECGs (electrocardiography) were done with LVH (left ventricular hypertrophy), T wave, and ST-T wave abnormalities. Chest X-ray was clear. I have been told that ECGs are normal. I will be doing a Holter monitor soon. I am overweight and taking Losartan 50 mg for blood pressure. I also have Hashimoto. These weird cardiac symptoms have been creeping up for maybe six months, and the past month has gotten worse. I am concerned about blood clots, mass, or something being missed. Please help.

The symptoms are likely to be related to heart rate fluctuations. We have to find out the cause of this. We will require tests like 2-D echocardiography, Holter, thyroid profile, serum creatinine, serum electrolytes, and LFT (liver function test). With hypertension, thyroid, excess weight, and sleep apnea, there are structural changes in the heart that may cause atrial fibrillation, atrial tachycardia, and excess ectopics. This may contribute to your symptoms. Treatment will depend on the cause of the symptoms. Blood clots may occur with irregular heart rates like atrial fibrillation. Heart mass may be diagnosed with 2-D echocardiography.

As I mentioned, ECG (electrocardiogram) findings are also related to heart structural changes. LVH (left ventricular hypertrophy) and left atrial enlargement are related to hypertension.

You did not provide the ECG (electrocardiogram) paper and provided interpretation only. My opinion is based on provided interpretation. ECG machine interpretation needs to be correlated with actual ECG. So these may be incidental findings.

Hello doctor,
I do not have the actual ECG printouts, just the interpretation. LFT done = ALT (alanine transaminase) is 94 U/L, and AST (aspartate aminotransferase) is 47 U/L. GFR (glomerular filtration rate) is 81, glucose is 108 mg/dL, creatinine and BUN (blood urea nitrogen) are normal, and TSH (thyroid stimulating hormone) and electrolytes are normal. Thank you.
