Hello doctor,
I am a 36-year-old male and have been using Tadalafil 20 mg SOS regularly for the past 1.5 years before sexual intercourse with my partner. The frequency of the medication is usually once every three to four days. Sometimes I take it once a week, or maybe once every two weeks, as needed. I do see a lot of improvement in my performance as a result of this.
Recently, I have been experiencing slight bloating and gas-like symptoms while consuming the medicine. I am undergoing treatment for GERD and have been prescribed Esomeprazole with Domperidone 40 mg and Itopride tablets for the past one week. The dosage is 10 days, and I have completed 1 week.
I do not have any history of heart disease, but I do suffer from sleep disruptions due to palpitations, which the gastroenterologist diagnosed as a severe acid reflux problem.
Is it safe to use Tadalafil SOS with these GERD medications?
Kindly advise if I can continue with Tadalafil. Are there any major side effects of Tadalafil in the long run?

Tadalafil is a prescription medication, and it should not be used unless indicated and advised by a certified physician. The most common use is for erectile dysfunction. The common side effects of the medication include:
Headache.
Nausea.
Vomiting.
Skin irritation.
Diarrhea.

The rare but more serious long-term side effects of the medication include:
Vision disturbances.
Severe allergic reactions.

No other serious long-term side effects are reported in the literature apart from those already mentioned. Tadalafil is not recommended in patients with cardiac issues, especially ischemic heart disease; however, it does not cause any cardiovascular issues by itself.
Bloating and indigestion are known side effects of the medication; however, if you were using Tadalafil previously without these side effects, then it is very unlikely that this issue is caused by consuming Tadalafil. Tadalafil can be used safely with these medications.
There is no direct interaction between Tadalafil, Esomeprazole, Domperidone, and Itopride that is considered clinically significant or potentially dangerous. However, if you feel any kind of health issue after using these medications, you can always contact your primary care provider.
The palpitations you are experiencing need further investigation if this has not already been done. If these are very frequent, I would recommend a 24-hour Holter monitoring. If this does not show anything pathological, then it will provide reassurance.
