I have black-spotted, tanned, dull, and sensitive skin issues on my face.

I read your query and understand your concern.

Dark spots, tanned and dull skin, and sensitivity, the main factors are mainly due to sun exposure, post-inflammatory hyperpigmentation, and a reactive skin barrier. Sunlight will worsen pigmentation and dullness, so daily broad-spectrum sunscreen (SPF 30–50) is essential, along with protective measures like hats and shade. Eat fresh fruits and vegetables. Avoid harsh scrubs and creams on the face.

I am giving you a regimen to manage this. Use a gentle cleanser and broad-spectrum SPF (sun protection factor) sunscreen. At night, use Aziderm cream pea-sized amount on the whole face, followed by moisturizer twice a week to remove the tan. On the remaining five nights, use Kojic acid and Arbutin cream at bedtime for whitening.

Follow this for one to two months to notice a big difference in skin texture, tanning, and marks. I hope this helps.

I can see (attachment removed to protect patient’s identity) hyperpigmented patches near your temples and around your lips, which are most consistent with melasma. Also, I need to rule out a fixed drug eruption.

Have you ever experienced pigmentation after taking any kind of antibiotic or painkiller?

Kindly let me know this, as the pigment of the fixed drug eruption is more resistant to treatment, and it may need other agents as well.

While for the melasma, I would like you to please get your serum iron, serum ferritin levels checked and get back to me so that the correction is also needed to further improve the pigment fast.

So, the treatment I have given you is already for melasma and tanning. It will improve your condition gradually, and also, I want you to get your labs done, and please answer me about the fixed drug eruption.
