Hello doctor,

I am seeking a second opinion on a ski injury from 3/10 that may involve a tibial fracture. The MRI from 3/14 is attached for your review.

Initial CT and X-ray at the hospital showed a tibial spine fracture, possibly extending to the tibial plateau, with lipohemarthrosis. X-ray, CT, and MRI images are attached for your review.

Thank you for your question. Although reviewing the X-rays was helpful (attachment removed to protect the identity of the patient). I can provide my initial assessment based on the MRI(magnetic resonance imaging) you provided.

The MRI indicates fractures in both the tibial spine and tibial plateau. Some images suggest the tibial spine fracture is displaced, and the joint surface is misaligned. An X-ray or CT(computed tomography) scan would clarify the extent of displacement and joint involvement.

Given these findings, this type of fracture is unlikely to heal properly without intervention. Persistent joint surface irregularity may cause early knee arthritis and restricted movement.

Surgical intervention will likely be necessary. This typically involves fixing the tibial spine with a screw and stabilizing the joint surface, possibly with plates, depending on imaging findings.
