Anything else you can say about it? I am currently taking Tadalafil 5 mg Atorvastatin 80 mg Metoprolol succ er 25 mg (None taken for 5 days before the test) Ecotrin 81 mg and Vitamin B-12

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

Please find my answers to your questions below.

1- It is a test that usually checks the autonomic nervous system interaction with cardiopulmonary function to maintain the essential blood pressure while changing the position from sitting to standing. Physicians usually request this test when someone comes with complaints of orthostatic changes, which means experiencing dizziness, confusion, light-headedness, or even a blackout during a rapid change of position from sitting to standing.

In such conditions, the underlying problem may be due to heart or vascular structural issues, blood volume or content abnormalities (for example severe anemia), or autonomic nervous system problems. The tilt table test can give us valuable information about the autonomic nervous system by monitoring the heart rate, blood pressure, and respiratory function during some maneuvers, especially during and after a position change.

a- Your test results of the sympathetic nervous system function were abnormal.

2a- The sympathetic nervous system is a complicated network of nerves that modulate organ reactions when a rapid response is needed. For example, during a potential situation of fight or escape. This system automatically increases the heart rate and vascular tone to increase the blood pressure. Similarly, during a change of position from sitting to standing, we need a rapid increase in blood pressure. This change is necessary for blood to be able to overcome gravity and reach the brain that its distance from the earth's surface has been elevated.

The test showed that in a simulated situation (the 70-degree tilt position), your blood pressure decreased, while it should have increased or at least remained stable. As a result, you would experience a transient shortage in the brain's blood supply that would manifest itself as a feeling of dizziness.

2b- To diagnose the possible underlying cause of this abnormal nervous system function, I need more data about your medical history and conducted lab tests. Nevertheless, with the currently available data, especially the skin biopsy and sudomotor test, the adverse effect of tadalafil may be regarded as a possible cause.

2c- It was discussed in 2a.

2d, e, and 3: Unfortunately, I cannot give you recommendations, predictions, or any advice with the limited data that I have about your medical history and other recent tests or paraclinical evaluations. I am sorry for this issue.

I hope I have answered your question.

Let me know if I can assist you further.

Regards.
