Hello doctor, I take 44 mg of Topiramate twice daily and also take Nexplanon. Does this medication increase the risk of pregnancy if my partner ejaculates inside me and makes Nexplanon ineffective?

Topiramate (anticonvulsant) causes a decrease in the serum etonogestrel (the active component in Nexplanon) level required to inhibit ovulation. However, this is dosage dependent. Topiramate of a daily cumulative dosage of 100 mg (milligrams) or less does not have a significant effect on the serum level of etonogestrel. So, since your daily cumulative dosage is 88 mg, it will not make your Nexplanon ineffective and it will not increase your risk of getting pregnant. However, if you want to increase your dosage of Topiramate above 100 mg per day, do let me know or your gynecologist, so that you can be reassessed and appropriate measures can be taken to prevent you from getting pregnant.
