Despite medications, what can cause my TSH levels to increase?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I was diagnosed with hypothyroidism two months back. My TSH level was 16 then, but my T3 and T4 levels were within normal limits. My doctor prescribed Thyronorm 25 mg to be taken daily on an empty stomach and instructed me not to eat for 45 minutes after taking the medication. Also, I was advised to take weekly vitamin D3 supplements.

I underwent another test, and my TSH level decreased to 6. I continued taking the same medication as prescribed. However, my TSH level has increased to 24. I am unsure why this has occurred despite faithfully following my medication routine. Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and understand your concern.

Based on your information, the high dose of vitamin D3 you have been taking may cause your elevated TSH (thyroid stimulating hormone) level. Vitamin D3 (cholecalciferol) can interfere with the absorption of thyroxine, which could affect your thyroid function. Considering your body weight, you should increase your daily dose to 50 mcg (microgram). Instead of waiting 45 minutes before breakfast, you should wait one hour after taking the medication. It is better to get daily sun exposure for vitamin D supplementation rather than relying solely on supplements, as they can interfere with absorption. Repeat your thyroid function test after three months, and we can adjust your medication dose if necessary. Titration of the dose is often required for various conditions, as it depends on how your body responds. It is essential to remain flexible in this regard.

I hope this helps.

Please feel free to ask if you have any further questions.

Thank you.
