Hello doctor,

Uneven skin tone, tanned skin. I have combination skin with an oily T-zone and also darkness around my mouth and sides of my nose. I suddenly go out in the sun, and my skin turns dark. What would you tell me to this? I also need a good whitening cream to get an even skin tone.

Thank you for your email. I know how frustrating it is to get uneven skin tone, tanning, darkness around the mouth and nose, especially when you step out of the sun, and your skin reacts. Don’t worry, this is a very common pigmentation concern and can be controlled well with the right routine and consistency.

According to your description, your skin seems to be a combination type (oily T-zone) with sun-induced tanning and perioral pigmentation. It is usually caused by too much melanin being made due to exposure to the sun, friction, and, in some cases, the sensitivity of the skin around the mouth and nose. The secret to improvement is gentle skin care, pigment-reducing creams, and strict sun protection.

Start by using a mild, simple face wash to clean your skin twice daily without stripping your skin of its natural barrier. Don’t use harsh soaps or scrubs, as they can make the pigmentation worse.

Apply Kojivit cream once a day, at night, on the whole face or on the uneven and darker areas. This cream helps to decrease pigmentation and to improve the overall skin tone over time. Apply Aziderm 15% (azelaic acid) only two nights a week. Apply a small amount the size of a pea to the pigmented areas, always under a moisturiser to reduce irritation. At the beginning, don't use Aziderm every night. Twice a week is enough and safer for your skin.

Then apply a soothing moisturiser like a ceramide or niacinamide moisturiser after you’ve applied your night treatment. Moisturising is very important as healthy, hydrated skin responds better to pigmentation treatments.

In the morning, after cleansing and moisturising, apply generous amounts of sunscreen SPF 50+ to the face, especially around the mouth, nose and T-zone. This is an important step. No pigmentation treatment will work without sunscreen. If you’re outdoors, reapply sunscreen every two to three hours, even on cloudy days.

The goal is brighter, more even-toned, healthy skin, and that takes time. If you stick to this regimen, you’ll begin to notice visible improvement within six to eight weeks and gradual brightening over three months.
