What should I do if my 3-month-old baby has a severe cough?

Hello doctor,

My 3-month-old baby girl had suddenly developed a mild cold. Today, she has started coughing severely. I tried steam inhalation and nasal drops, but the cough is making her restless and causing significant discomfort.

Kindly advise.

Cough and cold symptoms can make a young baby uncomfortable, and it is natural to worry when a three-month-old infant seems restless. Most coughs in infants are caused by a viral upper respiratory tract infection (URTI) and improve gradually with supportive care.

I would suggest the following:

  • Continue using normal saline nasal drops before feeds and sleep, then gently clear the nose if needed.

  • Feed the baby more frequently in smaller amounts to maintain adequate hydration.

  • Keep the baby's head slightly elevated while being held (do not use pillows in the crib).

  • Use a cool-mist humidifier in the room, if available.

  • Avoid exposure to smoke, strong perfumes, or other irritants around the baby.

Do not give over-the-counter (OTC) cough or cold medications to a three-month-old infant unless specifically prescribed by a doctor. Because the baby is only three months old, close monitoring is important.

I suggest you seek urgent medical evaluation if any of the following occur:

  • Fast or difficult breathing, chest indrawing (retractions), grunting, or wheezing.

  • Poor feeding or taking significantly less milk than usual.

  • Fever (temperature of 100.4° Fahrenheit or higher.

  • Excessive sleepiness, lethargy, or difficulty waking the baby.

  • Bluish discoloration of the lips or face.

  • Fewer wet diapers than usual.
