Hello doctor,
I am consulting for my wife. My wife was consulting a general physician for a routine check-up. As per her doctor, the urine analysis showed abnormal ratios. As per reports, there is blood in the urine. The second albumin-to-creatinine ratio dropped from the previous level of 22 to 7.50 mg/g, however, it is still high. Later the ultrasound revealed the presence of a right ureteric stone. I have consulted a kidney specialist and he told us to consult a urologist. As per the kidney specialist, there is hydronephrosis on the ultrasound report and it is causing back pressure on the kidneys. Kindly advise on the treatment course. I am attaching her blood tests and ultrasound reports. Kindly review.

According to the reports (attachment removed to protect the patient's identity), your wife has got a ureteric stone roughly around 8 mm. You can give a trial of alpha-blocker and diuretics for three weeks. Even if the stone does not come out, you can think of ureterorenoscopy surgery. Blood in urine is probably due to stones but can be due to other causes also. So better get a contrast CT scan (urogram) done to rule out other causes. She has got proteinuria, causing swollen legs. Is she diabetic?
