Hi doctor,

Six days ago, I had UroLift. Unfortunately, I cannot pee because of blood clots caused by the Urolift procedure. There was no blood or blood clots for the first three days. However, it began to appear on day four. In addition, I have been experiencing severe sharp pain and muscle aches in my right prostate since the first day of the procedure. It started to get better after day three, but I continued to experience sharp pain while urinating. There is no pain in the left prostate. On the other hand, my blood clots are growing in size, from 0.039 x 0.039 inch to 0.59 x 0.59 inch. I was able to pee out blood clots, but only recently did I experience blockage symptoms such as difficulty starting urination. What should I do to prevent this? Is this normal? Are there any remedies?

You are experiencing post-operative gross hematuria, blood clots, and pain in the right pelvis. Hematuria may occur following the UroLift procedure. It is manageable by taking hydration tablets. Take capsule Dutasteride (Avodart) once daily for three months. You can also take tablet Tranexamic acid (Cyklokapron) 500 mg daily until the bleeding stops. Please consult your doctor if you are taking anti-platelet or antithrombotic medication. A urine culture is also recommended because it could be an infection. Do not worry about pain. It will be gone in a few days. If you have severe pain, you can take a tablet of Brufen (Ibuprofen) 600 mg daily for three days.

Why is my right pelvic or groin pain so severe when I urinate? Is the right UroLift implant too tight, or is something wrong? Is this level of pain normal? How long will it continue? Today, my hematuria seemed to improve, but my urine flow decreased. My urine flow was strong for 5 to 6 days after UroLift, but now there is some block. As a result, the stream is becoming weaker and more difficult to initiate. Why is it? Will this get better with time? Is this due to my UroLift procedure? I do not have a fever, so I might not have any infections. I have read that most men experience mild symptoms after UroLift. Still, my symptoms appear severe given the intensity of my sharp pain that lasted seven days and three to four days of hematuria with consistent large blood clots.

I understand your concern.

Staples are placed under endoscopic vision during UroLift. It could be near the nerve on the right, causing you pain. Furthermore, because prostate cells are dying, there may be inflammation, and thus the flow is weaker than before. It would be best if you could give yourself another week for the inflammation to settle, and then I hope the pain would reduce and the flow would improve.
