Hello doctor,

I am 38 years old and have been suffering from chronic spontaneous urticaria for 11 months, with daily hives that are ruining my life. The wheals appear randomly throughout the day, usually measuring three to 1.1 to 1.9 inches in diameter, and are intensely itchy, lasting two to four hours before disappearing.

My total IgE is elevated at 220 IU/mL, and my C-reactive protein fluctuates between 8 and 15 mg/L during flares. I am taking Cetirizine 10 milligrams twice daily plus Ranitidine 150 milligrams twice daily, but I am still breaking out every day. My tryptase level is normal at 8.5 ng/mL, ruling out mastocytosis.

I have undergone extensive allergy testing, including food, environmental, and contact allergens, and all of which were negative. I tried Prednisone 40 milligrams for one week, which completely cleared the hives, but they returned immediately after stopping.

The hives often appear on my face and neck, which is embarrassing at work and social events. I have been to the ER twice when facial swelling accompanied the hives.

My dermatologist mentioned increasing antihistamines to four times the normal dose or trying Omalizumab injections. The constant itching is affecting my sleep and mood. I am scratching unconsciously and developing secondary infections. Are there other immunosuppressive medications I should consider? Will these hives ever go away permanently?

Living with daily hives for almost a year, especially when they affect your face and sleep, can be incredibly frustrating and exhausting. You have done the right steps so far, and it is clear you have been trying to understand the problem and find relief.

Since the standard antihistamines are not enough, increasing the dose (up to four times a day) is a common next step, and many people tolerate that well. But from my experience and in similar cases I have seen, Omalizumab injections can make a big difference. It works for many patients even when everything else has failed. Sometimes it heals within a few weeks.

There are other options, like Cyclosporine, if Omalizumab does not help, but it needs close follow-up due to side effects. It is essential to consult the doctor before changing the medicine. Also, some doctors try Montelukast or a short course of Dapsone, depending on the case and the labs.

Just one question I need to ask you. Did anyone check your ANA (antinuclear antibody) or thyroid antibodies? Sometimes, these hives are related to autoimmune issues, and that can guide treatment.

Most importantly, you are not alone in this. Many people go through similar struggles with CSU (chronic spontaneous urticaria), and although it feels endless now, it often settles over time or becomes manageable with the right treatment.
