I am 25. How can I manage vaginal dryness and itching?
Patient's Query
Hello doctor,
I am a 25-year-old female experiencing vaginal and vulvar dryness and itching. Scratching leads to swelling and soreness. The itching occurs in bouts throughout the day and worsens at night.
It is felt primarily around the vaginal area and not significantly on the external vulvar region. There is no vaginal discharge. These symptoms have been present for the past two weeks.
There is a history of recently switching laundry detergents. My current medications include a fluticasone nasal spray for allergies and an alginate syrup for laryngopharyngeal reflux.
Please advise.
Thank you.
Hello,
Welcome to icliniq.com.
I understand your concern, and I will do my best to help.
Your symptoms are most consistent with vulvovaginal irritation and allergic or contact dermatitis, likely triggered by the recent detergent change. This commonly causes dryness, itching, swelling after scratching, and nighttime worsening, without discharge.
Here are some of the most likely causes of vaginal and vulvar dryness and itching:
- Contact dermatitis or irritant reaction (detergents, soaps, fabric softeners, pads, wipes).
- Vulvovaginal dryness (can occur even at your age).
- Less likely: Yeast infection (usually has discharge) or bacterial infection (usually odor or discharge).
Please follow these steps:
- Stop the new detergent immediately and switch to fragrance-free, hypoallergenic products.
- Wash the vulva with water only (no soap inside or outside).
- Wear loose cotton underwear.
- Avoid scratching; keep nails short.
- Apply a bland emollient externally only (petroleum jelly or zinc oxide).
- A low-potency Hydrocortisone 1% cream externally once daily for three to five days can help with inflammation (do not insert vaginally).
See a doctor urgently if your symptoms persist for more than a week after stopping the detergent, if you develop discharge, odor, pain, fever, or skin cracks, and if the swelling becomes severe or painful.
If it does not improve, you may need an exam to rule out vulvovaginal dermatitis. Other less common causes (lichen conditions, hormonal dryness, or infection).
I hope this helps clarify things.
Take care.
