What causes a painful vulva swelling lasting 5 days in me?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have a painful, hard swelling around the vulva. It has been there for five days now. It started around the same time as my menstruation and keeps getting more swollen each day.

Kindly help.

Hello,

Welcome to icliniq.com.

I have read your query and understand your concerns.

Thank you for sharing those details. A painful, hard swelling around the vulva that has been worsening over several days is concerning and needs medical attention as soon as possible. Based on your description, possible causes include a Bartholin’s cyst or abscess, which happens when a blocked Bartholin’s gland near the vaginal opening forms a cyst that can become infected and turn into a painful abscess, showing symptoms like a hard, swollen, and painful lump near the vaginal opening, redness, warmth, difficulty walking or sitting, worsening pain during menstruation or sexual activity, and sometimes rupture with pus drainage. Another possibility is a vulvar infection or folliculitis, where ingrown hairs or infected hair follicles (especially after shaving or waxing) can form painful lumps, or a vulvar abscess due to bacterial infection. Hormonal-triggered swelling is also possible, as starting hormonal contraceptives such as Well Plan can sometimes disrupt local gland function and contribute to swelling or increased sensitivity. Five or more days of swelling and pain, especially with intermittent relief but recurrence, suggest a developing abscess or deeper infection, and over-the-counter painkillers like Ibuprofen only reduce inflammation without treating infection.

If it is an abscess, you may need antibiotics, possible drainage if the swelling is large or not responding, and a pelvic examination to confirm the diagnosis. While waiting for medical care, avoid squeezing or popping the swelling, apply a warm compress three to four times a day for 10 to 15 minutes to help a cyst drain naturally, avoid tight underwear, shaving, or sexual activity, stay hydrated, and keep the area clean with warm water (without soaps or harsh chemicals). Please visit a gynecologist, obstetrician-gynecologist (OB-GYN), or urgent care clinic as soon as possible, and if you experience fever or chills, pus discharge, spreading redness or swelling, or difficulty walking or urinating, go to an emergency room or urgent care immediately.

I hope this helps.

Thank you.
