Did my hormones cause my unexplained weight gain?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 39-year-old woman, and over the past five years, I have gained a lot of weight even though I have tried different diets and ways to eat healthier. I am now about 80 pounds heavier than I was in my twenties. I feel out of breath just by climbing stairs or doing small household work, and my knees and back hurt all the time because of this extra weight.

I feel tired most of the day, I snore loudly at night, and my recent blood tests show that I have high blood pressure and pre-diabetes. I also tend to eat more when I feel stressed or upset, and then I feel guilty about my weight and end up eating even more.

Could this weight gain be linked to some problem in my body, like slow metabolism or hormone imbalance, that is making it very hard for me to lose weight even when I try?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I read your query.

I truly understand how frustrating and exhausting this must feel for you. Gaining so much weight despite sincere efforts can be very discouraging, especially when it starts affecting your daily activities, energy levels, and confidence.

From what you have described and based on your symptoms and recent lab findings, it seems that your weight gain may not be due to diet and lifestyle alone. In many cases like yours, underlying medical or hormonal issues can play a major role and make weight loss much more difficult than expected.

One possible reason could be thyroid dysfunction, especially hypothyroidism, which slows down the metabolism and leads to fatigue, low energy, and gradual weight gain. Insulin resistance and pre-diabetes can also make it harder for your body to use sugar properly, which promotes fat storage and makes losing weight more challenging. In women, polycystic ovarian syndrome is another common cause of hormonal imbalance and weight gain.

Your loud snoring also raises the possibility of obstructive sleep apnea (a sleep problem where your breathing keeps stopping and starting while you are asleep), which can disturb sleep quality, increase daytime tiredness, worsen blood pressure, and contribute to further weight gain. Along with this, emotional or stress-related eating can reinforce the cycle you described, where stress leads to overeating and then feeling guilty leads to even more emotional eating.

To understand your condition better, it would be helpful to do some investigations such as blood tests including thyroid profile, fasting insulin, HbA1c (a blood test that shows your average blood sugar level over the last two to three months), lipid profile, and cortisol if required. A sleep study may also be useful to rule out obstructive sleep apnea.

Based on your symptoms, the overall picture suggests a metabolic and possible hormonal imbalance, along with emotional eating and suspected sleep-related issues, which together are making weight loss very difficult for you.

The treatment should focus on both lifestyle and medical support. This includes following a structured nutrition plan such as a low glycemic index diet, gradually increasing physical activity with low-impact exercises to protect your knees and back, and learning stress management techniques.

Medical support is also important, and you may benefit from a detailed metabolic evaluation by an endocrinologist. In some cases, weight-loss medications can be considered if lifestyle measures alone are not sufficient.

Going forward, maintaining a regular sleep routine, eating balanced meals with portion control, avoiding crash diets, and addressing emotional eating through counseling or behavioral therapy will be important preventive steps. Regular monitoring of your blood sugar, blood pressure, and weight will also help track progress.

Please follow up with me within the next two days with your investigation reports so we can plan a more targeted and effective approach for you. You are not alone in this, and with the right medical guidance and consistent support, sustainable weight loss is definitely achievable.

I hope this helps.

Kindly revert so I can assist you further.

Thank you.
