Introduction

The cell is a basic unit of the body. It constantly adjusts with the surrounding atmosphere and maintains a balance for normal functioning. These standard adjusting mechanisms are known as homeostasis. Homeostasis is an essential function of normal cell functioning. The cell responds to physiological or toxic stress by adapting to these changes, but when a cell can no longer adjust to the stress, it results in cell Injury. Cell Injury can be reversible; that is, it can return to its original state like chemical injury, anxiety, or trauma. All these can be reversed by cellular adaptation. Another cell injury is irreversible, which happens when a stressful stimulus is constant, and the cellular damage reaches a level where it can no longer go to a normal state—such injury results in cell death by necrosis and untimely apoptosis.

What Is the Common Pathway of Irreversible Cell Injury?

The common pathway of the irreversible cell injury traverses in the following sequence -

  1. Depletion of ATP - ATP, adenosine triphosphate is the main component from which the cell derives the energy. The constant trauma results in the depletion of these ATP energy stores, thereby leading to the failure of many cellular functions. The depletion of ATP results in the cellular and endoplasmic reticulum swelling. It also results in increased lactic acid production, which results in damaged cells and fatigue.

  2. Damage of Mitochondria - Due to the production of lactic acid, there is a lack of oxygen, which is called hypoxia. The cell mitochondria are damaged due to severe hypoxia and release the pro-apoptotic proteins, which hastens the process of cell death.

  3. Influx of Calcium - The increased intracellular calcium activity due to STP depletion leads to activation of the enzymes that break down the molecules inside the cells. These enzymes are destructive and result in cell death.

  4. Oxidative Stress - The cell injury pathways produce reactive oxygen species that damage the cellular components of the cell such as lipids, proteins, and DNA.

  5. Loss of Membrane - The damage to the integrity of cell membranes leads to the leaking of the contents, hastening the irreversible cell damage.

  6. Damage of DNA - The damage to DNA can also initiate the cell death pathway by depletion of ATP.

  7. Activation of Apoptotic Pathways - All these factors together lead to the activation of the apoptosis pathway, which leads to programmed cell death.

The irreversible cell death injuries have two endpoints that are called as:

  • Necrosis - Necrosis is a kind of uncontrolled cell death that results from acute cellular injury. Necrosis can be of various types:

    • Coagulation Necrosis - This kind of necrosis saves the tissue architecture, but the cells die and coagulate. A classic example is ischemia, such as myocardial infarction.

    • Liquidation Necrosis - This kind of necrosis in the brain and abscess, where the tissue becomes liquefied due to destructive enzymes and ATP depletion.

    • Caseous Necrosis- This type of irreversible cell injury is seen in tuberculosis, where tissue has a cheese-like appearance.

    • Fat Necrosis - This occurs in fat tissue due to persistent trauma. Fatty acids, along with the calcium deposits, form chalky depositions.

    • Gangrene Necrosis - This kind of necrosis usually occurs in large areas, such as a set of limbs, which happens due to loss of blood supply. It can be dry or liquefaction with bacterial infection. This necrosis is fatal and often leads to amputation of the affected limbs.

    • Fibrinoid Necrosis - This necrosis is associated with immune reactions in the blood vessels where the deposits of fibrins in the vessel walls are seen. This kind is seen in vessels of systemic lupus and malignant hypertension.

  • Apoptosis - Apoptosis is called programmed cell death, which is a biological process that ensures the controlled and efficient removal of the cells to maintain the functionality of the tissues and organs. The importance of apoptosis is

    • It is essential for proper development to eliminate unnecessary or improperly formed cells.

    • It helps to maintain homeostasis by removing aged and damaged or potentially harmful cells.

    • Apoptosis helps eliminate infected or cancerous cells.

While apoptosis is a physiological process, dysfunctional apoptosis can pave the way to diseases such as cancer, where insufficient apoptosis happens, or neurodegenerative disorders, where excessive apoptosis happens.

What Are the Causes of Irreversible Cell Injury?

The causes of irreversible cell injury encompass a variety of factors that can damage the cell significantly beyond the ability to recover. These factors include

  • Lack of Oxygen - Depleted or no oxygen in the tissues due to respiratory illness, severe anemia, or carbon dioxide poisoning can lead to cell death.

  • Reduced Blood Supply - This is often the result of blockage of blood vessels due to thrombosis or embolism and is commonly encountered in myocardial infarction and stroke.

  • Physical Trauma - The trauma from penetrative wounds, burns, frostbite, or heat stroke can lead to irreversible cell injury.

  • Chemical Causes - Environmental toxins such as heavy metals like lead and mercury, cyanide, or any drug overdose that leads to cell injury can cause liver failure.

  • Biological Factors - These factors include bacterial, viral, parasitic, or fungal infections. Also, excessive immune responses can lead to cell injury, which is often seen in autoimmune disorders.

  • Hereditary Factors - Genetic disorders like sickle cell anemia and cystic fibrosis can lead to irreversible cell damage. Mutations that are acquired from environmental factors can lead to cancerous cell injury and, ultimately, cell death.

  • Imbalance in Nutrition - Lack of nutrition, such as adequate vitamins and minerals, impairs cell functioning. Also, overnutrition can lead to cell injury due to metabolic syndromes.

  • Aging - The aging of cells leads to the accumulation of cellular damage over time and leads to impaired cell functioning and, ultimately, irreversible cell injury.
