Two strokes detected on the CT and MRI. How to deal with?

Hello, Welcome to icliniq.com. I have reviewed the MRI (magnetic resonance imaging) brain report (attachment removed to protect patient identity). I am sorry to say, but I do not agree with the diagnosis of subacute stroke as per the radiology report. The MRI that was done shows two lesions that are moderately large in size and do not follow the normal location for stroke. Also, we need other sequences, such as diffusion imaging, to say if it is a stroke or not.

Are these head-reeling episodes sue to stroke, seizure or syncope?

Hello, Welcome to icliniq.com. It is difficult to comment if the episodes are solely syncope or fainting spells due to orthostatic hypotension or abnormal heart rhythm. I agree with Holter monitoring. I would also recommend an EEG (electroencephalogram) study for one hour to make sure we are not dealing with seizures. Her hemoglobin is low and needs further workup regarding the cause.

Can epilepsy medications lead to memory impairment?

Hi, Welcome to icliniq.com. Thank you for your question. History of epilepsy and intermittent episodes of confusion with loss of memory of the episode (amnesia) is concerning for seizures. For the workup of seizures, he should get routine brain wave testing (EEG - electroencephalogram) or a long-term EEG monitoring to identify any abnormal brain discharges which can cause a seizure. Other things to look at it is head imaging like CT head or MRI brain (if not done already) as it is not uncommon for elderly patients to have blood collection around the brain (subdural hematoma) to present with altered awareness or confusion.

Yes, on the basis of the CT (computed tomography) of the brain, your father had a stroke on the right side of the brain which controls the left-sided symptoms. He had several other old strokes which probably did not cause any symptoms. The goal is to control the blood pressure better.
