What are the causes of five hypoglycemia episodes in a day?

Hi doctor,

My husband has type 1 diabetes. He has had no Insulin today but has had five hypos today, and his sugar levels are dropping again. He had his regular food all day, drank juice and cans of coke. He has not done anything different to any other day and also has not been stressed either. He treated all his hypos the way he normally would. What could be causing all these hypos if he has not had any fast acting Insulin since last night? Any advice would be greatly appreciated. Thank you.

What are the treatment options for Graves' disease when trying to get pregnant?

Hello doctor,

My sister is 32 years. After her second delivery, she got the problem of diffuse toxic goiter what is called Graves' disease. She is on medication for the last six years. She is on Carbimazole. But, it is not effective as when she stopped taking the medicine she suffers again from the symptoms (heart rate and sweating). The doctor advised her to get treatment by radioactive iodide. But she is worried this can lead to infertility. So, the other choice was to do a surgery to remove the thyroid gland. But, she will be on Thyroxine for the whole life. I wonder if this is the only choice she has. And, if this is the case, what is the best for her as she wants to get pregnant and she is breastfeeding now.

Note:

  1. The thyroid gland is diffusely enlarged with increased tracer uptake and homogeneous distribution.
  2. No definite focal lesion seen.
  3. 20 minute Tc uptake = 5.2 %.

My TSH level was 11.768 U/mL, do I have to start taking Synthroid again?

Hi doctor,

I had a thyroid test three years back, and my levels were 7.043 U/mL, so I was put on thyroid medication for hypothyroidism. After experiencing some side effects from the Synthroid drug, I discussed alternatives with an endocrinologist. He advised me to stop my medication and retest after six weeks. When I retested, my levels were 2.7 U/mL. My primary care doctor advised I test once per year, just in case. I had a test this month, and the levels were at 11.768 U/mL. I do not want to take pills again if this is just a random fluctuation. Why would my levels fluctuate this way and what do you suggest my course of action be? I started my period the day of my blood test, could that have affected the levels? Synthroid made my hair fall out, and I do not want to take it if I do not have to. Please advise.

What might have slowed down the rate of my alcohol metabolism?

Hello doctor,

I need help. We have zero-tolerance for alcohol at work. I had some alcohol eight hours (3 to 4 ounces) before work started. I am a 190-pound, good, shape male 5.8 feet and 48 years old. Based on the 1 ounce per hour metabolism, I was sure I would be in the clear. I do not have thyroid and realized I was cold and gaining weight. I had my TSH drawn, and I was hypothyroid. Could the hypothyroid state slow my alcohol metabolism? Any defense I have may help. I would never have drunk otherwise.

Will increased TSH level harm the baby's brain growth?

Hi doctor, I am 26 years old. My height is 5 feet inch and weight is 115.5 lbs. I am 20 weeks pregnant now. I underwent a surgery for complete thyroidectomy and I am taking Thyroxin 100 mcg regularly. In my first trimester, the TSH level was 0.31 and the doctor advised me to continue the same dosage of medicine. But, in my recent serum test, total T4 level is 10.5 mcg/dL and TSH has shooted to 14.37 mIU/L. The anomaly scan shows the baby is healthy with heartbeat of 153. But, I am worried about the TSH level which could harm the baby's brain growth. Doctor has prescribed 125 mcg of Thyronorm tablet for a month. Is it must to undergo an emergency thyroid check with an endocrinologist to avoid any complications?
