Why am I facing challenges in losing weight at the age of 22?

Hello doctor,

I am a 22-year-old female, and I have been facing challenges in losing weight at a pace comparable to how I gained it. Despite my best efforts during the summer, where I incorporated intermittent fasting, eliminated sugar, and focused on a diet of fresh and healthy foods for three months, I managed to lose only about 13.23 pounds. Unfortunately, due to unforeseen circumstances, I have regained 11.02 pounds. I am concerned that I might have a low metabolism and would like guidance on how to address this issue in a healthy and effective manner. Any advice on boosting metabolism and sustainable weight loss would be greatly appreciated .Kindly help.

Does low gamma globulin level affect HIV test results?

Hello doctor,

I had an unprotected relation two months ago, which led to urethritis three days after that was treated with a 10 days course of Levaquin. The urethritis disappeared, but at the end of the treatment, I started to feel neurological symptoms, numbness, tingling burning sensation in my arms and allodynia in my foot. Then a few weeks later, I began to experience diarrhea (muddy stools) after digestion (mainly in the morning, sometimes at night or the end of the afternoon). I lost a lot of weight (nearly 10 kg). I got an MRI (cerebral and medular) and an EMG that came back normal. I got STD (hepatitis, syphilis, HIV, gonorrhea, chlamydia) lab test 43 days after exposure, which came back negative. As I was worried about all these symptoms, I got multiple HIV fourth-generation testing at 21 days, 28, 35, 43, and 51 days that all came back negative.

My doctor gave me this week a complete blood test, and everything came back healthy except slightly high albumin level (53g/L) probably due to dehydration but also a somewhat low gamma globulin with a level of 7.4g/L while the lab norms range from 8 to 13.5 g/L. The conclusion stated hypogammaglobulinemia. Now I am worried because I read that hypogammaglobulinemia can delay the seroconversion and then give false negative antibody results. Do you know if such level of gamma globulin can indeed affect HIV testing?

Can oral antifungal agents help treat tinea versicolor?

Hello doctor,

I am a 26-year-old female, and I have developed raised white spots on my pubic area that are sometimes itchy. Despite never having had sexual intercourse, I previously visited a dermatologist for tinea versicolor on my back. I was prescribed Itraconazole for two weeks, followed by one dose of Fluconazole 200 mg after completing the Itraconazole treatment. Additionally, I have been applying topical Clotrimazole twice daily for one month on both my back and pubic area.

These white spots have persisted for more than eight years, and although the tinea versicolor on my back has cleared up, the raised white spots on my pubic area remain unaffected. Despite consistently applying Clotrimazole for over a month, there is no improvement. I am hesitant to seek medical attention outside of my home due to embarrassment. Could you please provide detailed advice on what I should take to resolve these persistent white spots?

What are the ways to recover from shingles?

Hello doctor,

I was diagnosed with shingles around two months ago. The scorching sensation has been incredibly acute, making it impossible to sleep. I felt relieved twice when they put me on Prednisone tapers for a month. However, once the Prednisone had left my system, the scorching sensation returned, and it was excruciating. I scheduled a doctor's appointment with my primary care doctor a month ago, but it was canceled yesterday since she would not be in the office, and now I am wondering what to do about the scorching pain. Because I have fibromyalgia, my system takes things quite hard. I am unable to tolerate the burning pain and tingling sensations in my hands, feet, fingers, and toes, and the fluid-filled blisters are filthy and leaking uncontrollably. I need a means to relieve the scorching agony since I cannot sleep or think well. It is just nasty. I am hoping there is something or anything that can be done to halt the constant burning so I can eat again without vomiting from the discomfort.

I presently take the following medications: Acetaminophen 500 mg, three pills twice a day as needed for discomfort from fibromyalgia, migraine headaches, and menstrual cramps. Albuterol sulfate HFA inhalation aerosol 90 mcg, two puffs daily as needed at nighttime for asthma; Amitriptyline 100 mg (Sandoz brand) For fibromyalgia and IBS, use one tablet daily before bedtime. Coenzyme Q10 200 mg, one Softgel twice daily to reduce high blood pressure; Diphenhydramine HC 25 mg, dye-free (Welly brand). Two tablets every six hours as needed for allergy control; D-Mannose 500 mg, three capsules twice daily for UTI prevention; Epinephrine auto-injector 0.3 mg (Amneal brand), injected once as needed for anaphylaxis reactions; and Levocetirizine dihydrochloride 5 mg, one tablet daily for allergies, hives, mucus, and cough. Loperamide hydrochloride 2 mg, two tablets daily as required for diarrhea caused by IBS 10.) Nortrel 28 per 1 mg-0.035 mg one tablet daily for PCOS (Polycystic ovary syndrome) and menorrhagia; Omeprazole 20 mg and Sodium bicarbonate 1,100 mg one capsule daily for GERD, esophagitis, hiatal hernia flare-up prevention, and heartburn prevention. Ondansetron orally disintegrating tablets USP 4 mg is one tablet taken twice a day to treat nausea and vomiting caused by chronic nausea, fibromyalgia, GERD, IBS, and migraines. Tolterodine tartrate, 2 mg, one tablet twice daily for vulvodynia discomfort and bladder leakage, and 12 days of Prednisone with 10 mg tablets.
