What is the normal titer value after typhoid?

Hello doctor,

I am 25 years old. I have a little long story of typhoid.

In March, I tested positive for typhoid with widal test and the report shows TO - 1:80, TH - 1:40. My doctor prescribed me some antibiotics and I took them for eight days. I tested again and all titers were negative. I immediately left all the medicines as I though I have recovered. I was fine.

In May, again I tested positive for widal test and the report shows TO - 1:160, TH - 1:80, TA - 1:40. This time I took two antibiotics Oflomac 200 and Ceftas 400 mg for about one month. After one month I tested for widal and my TO titer was 1:60 and TH was 1:40. My doctor said me to leave the medicines and he told that it is normal now.

Now here my concerns starts, in July and August, I took two widal tests on the interval of one month and both reports were the same as TO - 1:80, TH - 1:40. I showed this report to three doctors and they all said that it is normal. It will take some time to fall the value of titers after typhoid.

Symptoms:

I never had any fever during all history. I just had joint pains, leg cramps and sometimes stomach pain. My question is have I become a typhoid carrier or this is really normal? And if I have become the carrier then is it curable or not? I am very stressed about it and it has disturbed my whole life. I am asking this because, now these days, when I go for morning walks, I have leg pains whole day with a very bad taste of my tongue.

Can breast implant removal be done with local anesthesia?

Hello doctor,

I am 35 years old. I am having breast implant explantation surgery in four days. I have bad anxiety and the thought of anesthesia really scares me. I am a mother and the thought of being over sedated or not waking up at all is frightening. My surgeon has agreed to use local and MAC anesthesia, because I did not want general. Can they give me just light sedation so I am not totally out? If my surgeon would have agreed to just do local with a pain pill I would have been on board. Unfortunately, he did not go for that. I do trust my surgeon, he placed my implants almost 10 years ago and now the left implant is deflated (I have saline implants). I am also worried as when they asked if I smoked I was only thinking of cigarettes and said no. Then I called and told them I do smoke Marijuana daily. They told me to stop five days before. But I am worried now since reading online that some prefer two weeks clean. Will this cause me to be at an even greater risk now?

Also, I have a minor cold and cough, no fever but coughing some sputum up. Nothing major and stopping smoking weed could be a contributing factor now. I do not smoke cigarettes or drink alcohol. I have a history of congenital cervical spinal fusion (two discs), TMJ, and anxiety, panic attacks. I exercise regularly and I am on no other prescribed medication besides Xanax 0.5 mg at bedtime which I only actually take a half tablet of when needed and that does the trick for me. I take an Emergen-C immune plus vitamin pack daily and a Stress-B complex daily (which they have not told me to stop). I am an RN and have been trying to rationalize and reassure myself that it is a common procedure and others older and in worse condition have surgeries all the time and are fine. My mind frame is so different than it was 10 years ago, where I easily trusted surgery and after augmentation was fine. I am praying daily that I will just be fine and I am over thinking everything too much. Any insight and thoughts would be wonderful.

Why do I feel dizzy and light-headed when I stand up?

Hi doctor,

I am a 54 year old male. I do not smoke or drink. I do regular exercises. For the last seven years, I was suffering from sudden sleep disturbance with following symptoms such as breathlessness, palpitation and chest congestion. I underwent the following tests: ECG, chest X-ray, echocardiography, exercise stress test, pulmonary function test, VIDAS PL, MRI scan of brain and sleep apnea test. All the above tests were normal. At last, I was diagnosed as having acid reflux symptoms and accordingly on doctor's advise I took proton pump inhibitors for about four months (Topacid 40 mg, Nexapro 40 mg, and Perinorm CD). The medication is discontinued and with the change of lifestyle (like light dinner, breathing exercise, etc.) I was able to control the symptoms effectively. But, occasionally I was experiencing irregular heart rhythms and on a medical Specialist's advise I took Prolol TR 40 mg for two months and after a satisfactory result, the specialist advised me to discontinue the medicine. Now, the present episodes (as described below) compelled me to seek opinion and advice. Occasionally, I am feeling dizziness, breathing difficulty on a lighter side, blurriness on the eyes, chest congestion, etc. These symptoms are quite evident when I bent my head while doing exercise (not always). Last week, while doing a breathing exercise on a sitting posture and when suddenly changed the posture by getting up, I almost fainted, but recovered instantaneously. But afterward, I felt weakness in the legs and arms, cold, breathlessness, etc. I play badminton for one to two hours daily without any problem. My B-12 and hemoglobin levels are satisfactory and BP is normal in the range 85 to 130 Hg for the last five years without medication. There is no history of diabetes or cholesterol.
