What causes severe pain in the abdomen and back after sex?

Hello doctor,

I just finished having sex when I experienced a sudden, sharp pain in the lower right side of my abdomen. The pain has since spread across my entire lower abdomen and is now moving up toward the center of my stomach. I am also feeling pain in my lower back and around the tailbone. I am unable to stand or sit properly due to the severity of the discomfort. Additionally, I have a strong urge to urinate but cannot do so because of sharp pain, and I also feel the need to vomit but am unable to, again due to intense pain in my stomach and tailbone. I feel the urge to pass gas as well, but I cannot because of the same sharp abdominal pain. I am in serious pain and discomfort, and I have noticed that my abdomen has been bloated since the afternoon, accompanied by a general feeling of being unwell.

Based on the symptoms you have described, you may be experiencing pelvic inflammatory disease (PID), which is commonly caused by multiple microbial organisms. This condition could be associated with your recent sexual activity, so it would be helpful to know whether the intercourse was vaginal, anal, or oral.

To confirm the diagnosis and guide appropriate treatment, I recommend undergoing both a high vaginal swab and an endocervical swab test. Once you receive the results, please share them for further assessment. In the meantime, you may begin the following treatment: Ceftriaxone 1 gram administered intramuscularly once daily for 7 days; Doxycycline 100 milligrams taken orally, one tablet twice a day for 14 days; and Metronidazole 400 milligrams taken orally, two tablets three times a day for 14 days.
