Should I be concerned about my brother's angiomyolipoma?

Hi doctor,

My brother was diagnosed with renal angiomyolipoma two years ago, and a CT scan is done yearly to monitor the growth, and He is now admitted to the hospital due to severe right flank pain and leg pain before flank pain. His complete blood count is being monitored. CT scan with no contrast showed heterogenous focus in the right peripheric space may represent a hemorrhagic exophytic renal angiomyolipoma, cannot rule out post-traumatic or spontaneous bleeding into the right peripheric space, acute to subacute hemorrhage in the right retroperitoneal and intraabdominal regions, small left renal cortical cysts, atherosclerosis, and degenerative issues changes.

He was referred to a urologist. His BP is normal. My brother is also diabetic, with an HbA1c of 8.4 %. CT scan with contrast is not available in the remote hospital; CT was done without contrast. The blood test was performed, and it showed complete blood count, basal metabolic panel, hemoglobin- 13.8 g/dL, WBC- 19.0 per microliter, hematocrit- 41.5 %, platelets - 254 per mcL. Currently, he is taking medications for diabetes and high cholesterol. No medication was given for renal angiomyolipoma.

I understand your concern. It is angiomyolipoma and perirenal hematoma collection secondary to bleeding (attachment removed to protect the patient’s identity). I suggest the treatment should be by surgery and evacuation of the hematoma and removal of that angiomyolipoma to prevent further bleeding. The angiomyolipoma should be removed in total and sparring the corresponding kidney.

There is no need to repeat the CT (computed tomography) scan, but he needs monitoring and urgent surgery.
