Hello doctor,
I was diagnosed with ankylosing spondylitis a couple of years ago, and my recent tests showed normal inflammatory markers, which surprised my doctor, given my persistent lower back pain and limited mobility.
Despite the blood results, my pain has been affecting my daily activities significantly. I am also dealing with weight gain since I have been less active due to discomfort.
My doctor has been recommending physical therapy to help improve my range of motion, but I am skeptical about how much it can help. What are some of the effective ways to manage my pain and improve my mobility without worsening my condition?

Ankylosing spondylitis is an auto-immune condition linked to HLAB27 (a common gene in people with certain types of arthritis and other inflammatory diseases) and has a progressive course.
When left untreated you can suffer severe stiffness of joints, persistent pain, later on, difficulty in breathing, and generally loss of independent living.
You need to ask your doctor about biologics that are now available and more effective in improving long-term outcomes than just using anti-inflammatory tablets alone.
Also, I suggest you check with your rheumatologist (inflammatory disease specialist) about biological drugs like Etanercept, Golimumab, etc (biological TNF inhibitor)
These are more effective in controlling symptoms and improving outcomes. These drugs are not a cure though.
However, to stay productive you need to do:
A regular range of movement exercises.
Gentle stretches, and deep breathing exercises.
Yoga also has a role in staying flexible.
Regular exercises help improve muscle strength, improve cardiac and lung function, and allow for endorphin release that helps control pain.
Regularly stretching will keep joints supple.
Remember that the outcomes of treatment are still not guaranteed but with the biologics and regular exercises, you can stay active and independent.
