Hello doctor,
I am a 31-year-old man recently diagnosed with ankylosing spondylitis, and I am struggling to manage the symptoms. The pain in my lower back and hips has become nearly unbearable, especially in the mornings and after long periods of inactivity. It takes me almost an hour to loosen up each morning, and the stiffness returns if I sit for too long during the day.
I have noticed that the pain is starting to radiate into my upper back and neck as well. The NSAIDs prescribed initially are providing minimal relief, and I am concerned about long-term use. My sleep is significantly disrupted due to discomfort, and I am constantly tired. This condition is affecting every aspect of my life, and my work performance is suffering, and I have had to give up many of my hobbies. Are there any specific stretches or exercises you recommend to help ease the pain and maintain flexibility? Please help.

Staying active is one of the most important things you can do to manage AS (ankylosing spondylitis) (a type of arthritis that causes inflammation in the joints and ligaments of the spine):
Stretching and exercises to maintain flexibility:
Cat-cow stretch.
Pelvic tilts.
Bridge exercise.
Child’s pose.
Standing back extension.
Neck stretches.
Swimming can provide relief because the water supports your body weight while allowing you to move freely.
Poor posture can worsen AS symptoms, especially in the upper back and neck.
Make sure your workspace is ergonomically set up, and take frequent breaks to stretch and move around.
For sleep, you might find some relief by experimenting with different sleep positions, such as lying on your back with a pillow under your knees or on your side with a pillow between your legs to keep your spine in a neutral position.
A physical examination by your doctor in a hospital is a must.
