Hi doctor,
I am having back pain near the spine on the right side, especially when I go downstairs or go for a walk. My back gets locked when I am standing still. There is too much pain on the right side while sitting on a hard surface.
There is also nerve pain after having food sitting on the ground. There is pain only on the right side while using the commode. I have been suffering from this pain for the past two years and have not gotten any relief.

I have a feeling of continuous pain, and the pain is not of an electric type. The pain does not extend to the leg, but I experience pain in the right hip when I stretch backwards. The pain is not too hard to bear. The intensity of pain is about level three out of five. I do not have any tingling sensation or weakness in any leg. There has not been much increase in pain.
I am a software development professional, but I do not sit for more than half an hour continuously. I do take breaks after half an hour of sitting. I had no injuries previously. I am also having pain in my heels and ankles, so I cannot stand for long. I cannot sit without support because of pain. Mostly, I have to sit straight with support because when I bend, there is pain in my spine. It looks like my back joints are added to each other. There is pain in the place where the spine ends on the right side. There is also pain while bending the spine in front. Let me know if you notice any problems in the MRI.

It looks like there is a problem in your sacroiliac joint on the right side. It is not due to nerve compression. The exact diagnosis would be possible only after your clinical examination. The MRI images are not very clear. However, there is no problem that is evident on the MRI in relation to your complaints. So, it looks like a sacroiliac joint arthritis.
You need to start exercising for the same, and for this, you will require a physiotherapy consultation. By that time, you can start on a short course of painkillers. Even after adequate physiotherapy for about a month, if there is no improvement, then you might need to take local injection therapy for pain. Do not lift weights and do not bend your back. Also, avoid prolonged sitting.
