Hello doctor,
I have a red area that is itching on the penis head for a week. It looks like balanitis according to the pictures I searched. I also have pain after urinating. I have tried with Nystatin cream, but I have more itching than before. I cannot go to the ER, but the quickest appointment with the specialist can be scheduled in a week.
Please help.

I have read your queries.
It looks like severe candidal balanoposthitis (attachment removed to protect patient identity). Another possibility can be lichen planus or FDE (fixed drug eruption). But both of these conditions have more pain than itching.
So take the following treatment for candidal balanoposthitis.
1. Tablet Fluconazole 200 mg one tablet stat and repeat after one week. Similar treatment for the sexual partner.
2. Tablet Levocetirizine 5 mg twice a day for a week.
3. Apply Clotrimazole cream twice a day for a week.
Is there any history of drug intake before the onset of this? Are there any other lesions on the body or oral mucosa?
Start the medications and review after a week.
