Hi doctor,

The foreskin of my penis at the periphery is rupturing, with pain when I retract the foreskin. I also have itching, just at the circumference of the penis forehead, where the foreskin is completely retracted and below the urine hole.

I am not circumcised. There are also some rashes on the left side of the scrotal sac. I have been using Clotrimazole ointment on the penis head and Clotrimazole dusting powder around the scrotal sack. Please help.

The condition you describe is known as balanoposthitis. It is an inflammatory condition of the foreskin and head of the penis that is common among men with poorly controlled diabetes (explain). It usually comes and goes from time to time and heals with the use of steroid creams such as Surfaz-SN cream (Beclometasone 0.025 percent w/w, Neomycin 3500 u/1g, and Clotrimazole 1 percent w/w). Consult your doctor and use the medicine accordingly.

However, if you are repeatedly having this problem two to three times a year, your best option would be to get a circumcision done to remove the foreskin. The most important aspect of managing this is strict diabetes control (if you have not been evaluated for diabetes, please get it done).

To summarize,

1. For an occasional breakout (once or twice a year), steroid creams for five to seven days will help. Please note that repeated use of these creams may thin out the skin.

2. For recurrent breakouts of balanoposthitis, circumcision is the best solution (especially if it interferes with intercourse).
