Why did I have swelling in the urinary area for 3 months?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I have had swelling in the urinary area for three months, and it has not improved at all. My periods are normal.

Please advise.

Hi,

Welcome to icliniq.com.

I understand your concern.

Swelling in the urinary area for three months that has not gone away is not something to ignore. Since your periods are normal, this looks more localized to the vulva or urethra area rather than hormonal.

Because it has been persisting for months, you should definitely get it examined physically by a gynecologist.

The probable causes may be any of the following:

  1. Bartholin’s gland swelling.

  2. Chronic vulval infection.

  3. Growth or urethral prolapse (less common).

  4. Urethral caruncle.

  5. Localized varicosities.

I would advise you to get the following investigations done:

  • Local physical examination by a gynecologist.

  • Pelvic ultrasound (in case any lump or cyst is suspected).

  • Urine routine and culture.

  • Swab of discharge (if present).

The differential diagnosis will include:

  1. Bartholin’s cyst or abscess.

  2. Vulval lipoma or fibroma.

  3. Urethral caruncle or prolapse.

  4. Vulval varicosity.

The most probable diagnosis in your case seems to be chronic vulval or Bartholin’s cyst swelling.

I would recommend that you get a local examination and possibly in-office surgical drainage (in case there is a cyst). Also, if there is any infection, you may need antibiotics after culture. Since it is persistent, it will not resolve with home remedies. Warm sitz baths do help temporarily, but please do not delay the consultation.

You may follow the following preventive measures:

  • Maintain local hygiene.

  • Avoid tight clothing.

  • Keep the area dry, avoid constant moisture.

  • Regular gynecological check for recurrence.

Please share if there is pain, discharge, or any associated fever. Upload a photo if possible, and the urine report if done. It is best to see a gynecologist in person for a proper exam. You will need to follow up with me here and also have a direct specialist examination for confirmation.

I hope this answers your query.

Thank you.
