Hi doctor,

Two months back, I had an angiogram of head and neck, which came back negative. Is it possible to develop a pulmonary embolism just two months after the angiogram? For the past several days, I have back pain. What other symptoms can I expect?

I understand your concern.

Your symptoms are not suggestive of pulmonary embolism as symptoms of this disorder are much more dramatic and acute. On the other hand, angiography does not lead to pulmonary embolism. In my opinion, the back pain could be related to a wrong posture or chronic degeneration in the cervical and thoracic vertebral column. So, I suggest you a vertebral column latero-lateral x-ray study for achieving a proper diagnosis.

Thank you for your response.

I have pulsatile tinnitus, so I also had an MRV and lots of other tests. Can you explain to me the result from MRV- “congenitally hypoplastic or absent left transverse sinus, make a diagnosis of intracranial hypertension a more likely etiology of her right pulsatile tinnitus. We would like to obtain a dedicated CTV to evaluate for transverse sinus stenosis.” Is this serious?

These MRV (magnetic resonance venography) findings indicate an abnormality in the venous transverse sinus by birth, which is a normal space where the venous blood is drained from the brain.

This may sometimes lead to intracranial hypertension- which would cause headaches similar to migraine (throbbing headaches with nausea), more prominent during lying down, and improved standing up. Do you get these symptoms?

I would suggest consulting with an ophthalmologist to exclude papilledema (swelling of the optic nerve), closely related to intracranial hypertension. If this test is negative, there is no reason to worry about it.

Thank you for your kind words.

But I have a few more questions-

1) When I had a brain angiogram procedure, it would have shown the presence of venous sinus stenosis?

2) Also, during the angiogram procedure of the brain and neck, I could feel something hurting around my neck. I let the lab technicians know, and they fixed it. Is it normal for this to happen during this procedure? That is why I wondered if I had a clot or embolism in my back.

Firstly yes, the brain angiography would show the transverse sinus stenosis if it was present at that time.

Secondly, It is quite normal to have some discomfort in the neck during the angiography procedure. I would completely exclude any clot or embolism in your back based on your description. So please try to relax and do not worry about it. In case of any further query, do revert.
