Hi doctor,

I am 50 years old and recently started experiencing frequent urination and occasional discomfort. I am concerned these might be early signs of prostate issues, possibly even prostate cancer. Should I be screened, and if so, what tests would give a clear picture of my prostate health? Are there lifestyle changes I can make to lower my risk, and how often should I be checked at my age?

I have gone through your query, and frequent urination can be due to different issues. You are just 50 years old, so mild prostatomegaly can be one of the reasons, but not cancer. It can be due to urinary tract infections, diabetes, or high blood sugar. Be calm, get yourself tested, and follow a proper diet.

The probable causes include infection, prostatomegaly, or high blood sugar. Investigations to be done include:

  1. Blood sugar levels - fasting and postprandial glucose.

  2. Glycated hemoglobin, glycosylated hemoglobin (Haemoglobin A1c) (HbA1c).

  3. Complete blood count (CBC).

  4. Liver function test (LFT),

  5. Kidney function test (KFT).

  6. Urine routine, microscopy (R/M), and culture sensitivity (C/S) test.

  7. Ultrasound of kidney, ureters, and bladder (USG KUB).

The differential diagnoses include urinary tract infection (UTI) and diabetes.

The probable diagnosis is urinary tract infection (UTI).

The treatment plan includes

  1. Tablet. Nitrofurantoin 100 twice daily (BD) for 7 days.

  2. Tablet. Zincovit (multivitamin with Zinc) once daily (OD) for 10 days.

  3. Syrup. Citralka (Disodium hydrogen citrate) 10ml in 100 ml lukewarm water thrice daily (TDS).

  4. Capsule. Pan 40 mg (Pantoprazole) for 10 days.

  5. Tablet. Urispas (Flavoxate hydrochloride) sos if painful urination

The preventive measures include:

  1. Drinking plenty of fluid.

  2. Avoiding smoking (tobacco intake may lead to prostatomegaly).

  3. Taking a protein-rich fiber diet.

Regular follow-up is to be done with reports.
