Hello doctor,

I am a 57-year-old male with a long-term history of depression with OCD, bipolar disorder, and self-diagnosed with a borderline personality disorder. I am taking tablet Tegretol 400 mg and Paxil 60 mg. I have had remarkable effects with it, so I continued taking Paxil and Tegretol three months ago. I was doing fine, but then had a life event where I had a sexual experience that turned me immensely. It gave me substantial obsessive cycling, and I cannot stop thinking about it. It resulted in constant masturbation. I feel hideous and depressed. I cannot stay focused on my duties due to the ruminating thoughts and feelings. I am a travel nurse, and so I am away from my partner. Should I restart taking tablets Paxil and Tegretol, or is there something newer and better?

I can completely understand your current situation. You are having a relapse of OCD (obsessive-compulsive disorder) triggered by a sexual incident. I would suggest restarting Paxil (Paroxetine) and Tegretol (Carbamazepine). However, doses should be smaller initially and increase gradually after consulting your doctor. Ketamine is a good antidepressant, but the effects will stay only for a week or less. These ruminating thoughts are generally resistant to Ketamine.
